Students referred to examination (referral is sent directly to the Family Medicine Centre) after:
1. completing the Preventive Examination Card. on-line
2. completing Laboratory examination and/or sanitary-epidemiological examination – results of those tests will be delivered directly to the Family Medicine Centre.

Student will be referred to an appointment with an occupational physician after booking an appointment through e-mail sent to studentcmr@gumed.edu.pl at:

Family Medicine Centre of the Medical University of Gdańsk Debinki 7 Str, 80-211, Gdańsk – at the building at the photo above

Student needs to appear at 11:30 AM or 12:30 PM (as scheduled) in room 7 (from Tuesday to Thursday).

During the visit, the doctor, on the basis of the patients history (data included in the Preventive Examination Card), analysis of the provided results (depending on the major) and physical examination, will be able to issue a certificate of lack/existence of contraindications to taking up studies.

Medical certificate excluding/indicating contraindications to taking up studies as well as the Certificate from sanitary-epidemiological examination issued by the Family Medicine Centre will be sent to the person concerned and to the appropriate Dean’s Office.
